Schistosomiasis, the widespread neglected tropical parasitic disease transmitted by freshwater snails, is broadly categorized into two major clinical forms: intestinal schistosomiasis and urogenital schistosomiasis. Both variants are deeply intertwined with the livelihoods of marginalized groups, disproportionately affecting poor and rural communities across tropical and subtropical regions, with the highest burden falling on agricultural workers, inland fishing populations, and children who regularly play in unprotected natural water sources. Unlike many diseases that spread through isolated outbreaks, these two forms of bilharzia are often deeply embedded in local daily routines: farmers wading through contaminated irrigation channels to tend crops, fishermen spending hours waist-deep in lakes to cast nets, and women washing household items at unimproved riverbanks all face repeated, long-term exposure that sustains persistent transmission in endemic areas.
Intestinal schistosomiasis, most commonly caused by the species Schistosoma mansoni and Schistosoma japonicum, targets the blood vessels lining the intestinal wall, triggering a cascade of symptoms that often develop slowly over months or even years of untreated infection. Early-stage cases frequently present with persistent abdominal pain, recurring bouts of diarrhoea, and visible traces of blood in the stool—symptoms that are often dismissed as mild gastrointestinal discomfort at first, leading many patients to delay seeking medical care. As the infection progresses, the massive number of parasite eggs trapped in the intestinal and surrounding liver tissues triggers chronic inflammatory responses, forming clusters of immune cells called granulomas that gradually scar organ tissue. Liver enlargement becomes a defining sign of advanced intestinal schistosomiasis, a condition so common in high-prevalence rural regions that local communities often recognize it as a distinct, long-term health complaint rather than a random illness. This progressive liver damage is frequently followed by dangerous complications, including the accumulation of fluid in the peritoneal cavity known as ascites, and severe hypertension in the abdominal portal blood vessels that carry nutrient-rich blood from the digestive system to the liver. In these late-stage cases, the abnormal pressure in the circulatory system also forces the spleen to swell dramatically, leading to hepato-splenomegaly that drastically impairs the body’s ability to filter blood and fight off other common infections.
Urogenital schistosomiasis, caused almost exclusively by Schistosoma haematobium, targets the blood vessels of the bladder and urinary tract, creating a completely distinct set of health impacts that are often overlooked in basic public health screenings. Its most recognizable early symptom is blood in the urine, a sign that was once so common in endemic African villages that it was mistakenly seen as a normal part of adolescent development for generations. Over time, the eggs trapped in the bladder wall cause repeated inflammation, leading to chronic ulcers, bladder wall thickening, and an elevated long-term risk of squamous cell carcinoma of the bladder. The infection can also spread to the ureters, kidneys, and reproductive organs, causing painful urination, recurrent urinary tract infections, kidney damage, and irreversible fertility complications for both men and women. For women, untreated urogenital schistosomiasis can lead to genital lesions that drastically increase vulnerability to other sexually transmitted infections, creating hidden health burdens that extend far beyond the initial parasitic infection.
Beyond these two dominant forms, rare atypical cases can occur when stray parasite eggs travel to the lungs, nervous system, or other distant organs, triggering life-threatening complications that are far harder to diagnose and treat. Together, these different forms of schistosomiasis create a layered public health challenge that demands targeted, community-specific prevention, screening, and treatment strategies to reduce the long-term suffering of the world’s most vulnerable populations.
