Public-Education Series: What Are the Symptoms of Dengue Fever and Severe Dengue?

Dengue fever typically begins after an incubation period of 4 to 10 days following the bite of an infected mosquito, with symptoms that closely resemble a severe flu but follow a distinct and often debilitating progression over 2 to 7 days. The illness almost always starts abruptly with a high fever, often spiking as high as 40°C (104°F), which is usually accompanied by at least two of the following hallmark symptoms:

 

Intense headaches that feel like a constant, pressing pain behind the forehead and temples, often resistant to common pain relievers.

Deep pain behind the eyes, which tends to worsen with eye movement or gentle pressure, giving many patients a characteristic heavy-eyed appearance.

Severe muscle, joint, and bone pains that have earned dengue its historical nickname breakbone fever, with patients describing a sensation as if their bones are being crushed or twisted.

Nausea and vomiting, which can lead to dehydration and electrolyte imbalance if oral fluids cannot be kept down.

Swollen glands, particularly in the neck and groin areas, as the bodys immune system mounts a response to the virus.

A distinctive skin rash that may appear in two phases: an initial flushed redness across the face, neck, and chest in the early days, followed by a secondary pinpoint rash that spreads across the torso, arms, and legs as the fever begins to subside.

During this febrile phase, most patients experience extreme fatigue and loss of appetite, making even simple daily activities exhausting. While the majority of dengue cases are self-limiting and resolve within a week, a small but significant proportion of patientsespecially those experiencing a second infection with a different dengue serotypeenter a critical transition phase that can rapidly escalate to severe dengue.

 

Severe Dengue: Recognizing the Warning Signs Before It’s Too Late

 

The progression from dengue fever to severe dengue typically occurs around 3 to 7 days after the first symptoms appear, often just as the initial high fever starts to drop. This decrease in temperature is a dangerous false sign of recovery; instead, it marks the beginning of the critical phase where the following warning signs demand immediate medical attention:

 

Severe, persistent abdominal pain that feels like constant pressure or tenderness, often concentrated in the upper right abdomen below the rib cage.

Frequent vomiting (three or more times within 24 hours) that may prevent keeping down even small sips of water.

Bleeding from mucous membranes, such as bleeding gums, nosebleeds, or easy bruising with minimal contact.

Rapid, shallow breathing or feeling short of breath even while resting.

Persistent fatigue combined with restlessness or irritability, especially in children who may become unusually listless or agitated.

Blood in vomit or stool, which appears as dark, coffee-ground-like material in vomit or black, tarry stools.

Intense, unquenchable thirst despite drinking fluids, as plasma leakage reduces blood volume.

Pale, cold, or clammy skin, particularly in the hands and feet, indicating poor circulation.

When these warning signs are missed or ignored, severe dengue can lead to three life-threatening complications:

 

Plasma leakage syndrome, where blood vessels become excessively permeable, causing fluid to leak into the chest and abdominal cavities. This can lead to potentially fatal shock (dengue shock syndrome) and/or respiratory distress as fluid accumulates around the lungs.

Severe bleeding from the gastrointestinal tract, brain, or other internal organs, often triggered by a combination of low platelet counts and impaired blood clotting.

Severe organ impairment, particularly affecting the liver (causing severe hepatitis), heart (leading to myocarditis), or brain (resulting in encephalopathy or seizures).

For anyone showing these severe symptoms, hospitalization is essential for careful fluid management, continuous monitoring, and supportive careinterventions that have dramatically reduced dengue mortality rates in regions with strong healthcare systems. Public awareness of these symptom progressions, especially the deceptive fever drop that precedes the critical phase, remains one of the most effective tools for preventing severe dengue fatalities worldwide.
