Public-Education Series: Who Spreads Dengue and Severe Dengue?

Dengue fever is not contagious between humans through casual contact, coughing, or shared surfaces—its entire transmission cycle depends on a specific, highly efficient mosquito vector: the female Aedes aegypti. This urban-adapted mosquito becomes infected when she takes a blood meal from a person who is in the viremic phase of dengue infection, typically from one day before symptoms appear through the first 4–5 days of illness. Once the virus enters the mosquito’s body, it replicates in her midgut and salivary glands over approximately one week (extrinsic incubation period), after which she becomes capable of transmitting the virus to every new person she bites for the rest of her lifespan, which can last several weeks under favorable conditions.

 

Unlike many other mosquito species that prefer rural or forested environments, Aedes aegypti thrives in densely populated human settlements. She is a remarkably domestic mosquito, rarely venturing more than 400 metres from her breeding site throughout her entire life. This limited flight range, however, is offset by her exceptional ability to exploit human habitats: she lays her eggs not in large natural water bodies, but in small, artificial water-filled containers commonly found around homesdiscarded tires, flowerpot saucers, water storage jars, blocked gutters, and even bottle caps. This adaptation keeps her persistently close to people, turning every household with stagnant water into a potential dengue hotspot.

 

Daytime Biting: An Unseen Threat in Daily Life

Aedes aegypti is primarily a daytime feeder, with peak biting activity occurring during two key periods: early morning (just after sunrise) and late afternoon (before dusk). This behavioral trait fundamentally shapes human exposure risk: unlike malaria-carrying mosquitoes that bite at night when people are under bed nets, Aedes bites when people are most active and least protectedduring commuting, outdoor work, school activities, or relaxing in yards and courtyards. Consequently, a person can be bitten not only at home but also at workplaces, schools, markets, or public parks, making personal bite protection a round-the-clock necessity in endemic areas.

 

The Human Role: Amplifiers and Carriers

While dengue cannot spread directly from person to person, infected humans play the central role in sustaining and expanding transmission in two critical ways:

 

Amplifying the Outbreak: An infected person circulates the virus in their bloodstream for several days, during which any biting Aedes aegypti can acquire the virus and later transmit it to others. This mosquitohumanmosquito cycle can quickly turn a single imported case into a local cluster outbreak.

 

Carrying the Virus Across Borders: Infected travelersoften unaware they are ill during the incubation periodcan carry the virus from one country or region to another. When they are bitten by local Aedes mosquitoes in a new area, they introduce the virus into previously unaffected mosquito populations, potentially triggering outbreaks far from the original source. This travel-associated dengue has been a key driver behind the geographic expansion of the disease into temperate regions like southern Europe and the southern United States.

 

Why Aedes aegypti Is Such an Efficient Epidemic Vector

Several behavioral adaptations make Aedes aegypti exceptionally effective at spreading dengue:

 

Intermittent, Multiple-Host Feeding: Unlike mosquitoes that take a full blood meal from one person, Aedes aegypti often bites several people in a single feeding cycle, probing or partially feeding on one person before moving to another. This multiple biting behavior dramatically increases the number of people a single infected mosquito can infect in a short period.

 

Synanthropic Preference: She strongly prefers human blood over animal blood and lives in close association with human dwellings, ensuring frequent human-mosquito contact.

 

Egg Survival: Her eggs can withstand drying for months, hatching only when submerged in water, allowing populations to rebound quickly after dry spells.

 

While Aedes aegypti is the primary global vector, its relative Aedes albopictus (Asian tiger mosquito) also transmits dengue and poses an additional threat due to its greater cold tolerance and ability to breed in more varied habitats, including vegetated areas. Together, these mosquitoes, combined with human mobility and urbanization, have turned dengue into a persistently expanding global health challenge.
