Leishmaniasis Control Strategies
Context-Adapted Core Framework
Leishmaniasis control is a highly customized public health system project that must be tailored to local conditions, rather than adopting a one-size-fits-all universal model. Key influencing factors include the local epidemiological profile, geographical characteristics of transmission habitats, identified parasite and vector species, the presence or absence of animal reservoir hosts, regional ecological features, co-circulation of other vector-borne diseases, and complex social contexts such as large-scale population displacement, rapid unplanned urbanization and deforestation that expand sandfly breeding ranges. This context-dependent design ensures control measures can precisely target local transmission weak points, rather than wasting limited public health resources on ineffective interventions.
Five Core Intervention Pillars
The global WHO-endorsed leishmaniasis control system consists of five mutually supportive core strategic components, each with proven real-world effectiveness in endemic regions.
1) Early case detection and treatment: Deploy rK39 rapid diagnostic tests at primary health care facilities to identify suspected cases within 24 hours, and initiate standardized full-course antileishmanial therapy under medical supervision. This not only reduces the 95% untreated mortality rate of visceral leishmaniasis, but also cuts off the parasite transmission chain from human hosts to sandflies.
2) Reservoir host management: In zoonotic endemic zones where domestic dogs are the primary reservoir, conduct regular serological screening for all registered dogs, isolate and treat positive cases, and properly manage stray dog populations to eliminate hidden parasite sources.
3) Targeted vector control: Carry out indoor residual spraying of pyrethroid insecticides on residential walls during the peak sandfly activity season from May to September, and distribute long-lasting insecticide-impregnated bed nets to all households in high-risk villages to reduce indoor sandfly biting rates by over 80%.
4) Environmental management: Organize community clean-up campaigns to remove accumulated organic waste, fill wall cracks, and regularly clean livestock shelters to eliminate the dark, humid breeding sites preferred by phlebotomine sandflies.
5) Social determinant improvement: Upgrade rural residential housing conditions, popularize basic personal protection knowledge, and provide nutritional support for vulnerable groups to reduce population susceptibility to symptomatic infection.
Proven Real-World Outcomes
This integrated control strategy has delivered remarkable results in multiple historic endemic regions. In eastern China, the human-type visceral leishmaniasis transmission chain was fully interrupted by the 1980s through coordinated case management and vector control, eliminating local indigenous transmission in the vast plain areas. Even in the more challenging animal-source mountainous and desert endemic zones in western China, sustained implementation of these five strategies has kept local case numbers at a historically low stable level, effectively preventing large-scale outbreaks.
