Core Contamination Logic

The entire STH transmission cycle is tightly linked to poor sanitation infrastructure in under-resourced communities. Infected people shed thousands of STH parasite eggs in their feces every day, and in areas without properly constructed latrine systems, human waste is directly discharged into the surrounding environment, contaminating village soil, farmland and residential yards. Under suitable warm, humid conditions with temperatures between 22℃ and 30℃, the eggs take 2 to 4 weeks to mature into infective stages that can survive in the soil for up to 12 months, remaining infectious even after short periods of dry weather.

Fecal-Oral Infection Routes

The vast majority of STH infections are acquired through the fecal-oral pathway, which accounts for over 70% of all recorded cases globally.

Parasite eggs stick to the surface of vegetables grown on contaminated farmland; if these vegetables are not thoroughly washed, peeled or fully cooked before consumption, the ingested eggs will hatch in the human intestine and develop into mature adult worms.

Young children who frequently play barehanded in contaminated soil often put unwashed fingers into their mouths, directly ingesting STH eggs stuck to their skin, making children aged 5 to 14 the highest-risk group for roundworm and whipworm infections.

Drinking untreated water that has been contaminated with STH eggs from nearby soil or fecal waste is another common infection route in remote rural areas without centralized water purification systems.

Skin Penetration Route for Hookworms

Unlike roundworms and whipworms that only spread via oral ingestion, hookworm larvae that hatch in contaminated soil can actively penetrate intact human skin. This infection pathway almost exclusively occurs when people walk barefoot on polluted soil, as the tiny, motile larvae can burrow through the soft skin on the soles of the feet, enter the human bloodstream, and eventually travel to the small intestine to mature into adult hookworms. This transmission route explains why hookworm infection prevalence is often highest among working-age adults who spend long hours working barefoot on farmland.

Hidden Transmission Risks

Many people carry asymptomatic STH infections without obvious clinical symptoms, but they still shed large numbers of parasite eggs in their feces, acting as hidden reservoirs that continuously sustain local environmental contamination. This makes it impossible to fully break the transmission chain without coordinated improvements to sanitation, personal hygiene habits and community-wide deworming programs.
