Public Education Series: What Are the Control Strategies for STH?

The foundational and most cost-effective long-term control strategy for soil-transmitted helminths is systematic fecal management, which targets the root of STH transmission by preventing untreated human feces from contaminating residential soil, farmland and water sources. Proper composting and harmless treatment of feces can kill over 90% of STH eggs within 2 weeks, breaking the 3-week maturation cycle required for eggs to develop into infectious stages in the soil. Regular helminthological testing of topsoil in high-risk areas such as kindergartens, farmland and residential yards can accurately assess local contamination levels, guiding targeted decontamination measures to reduce environmental exposure risks.

 

Population-wide preventive chemotherapy is the core immediate intervention recommended by WHO for STH endemic regions. Distributing the four WHO-verified anthelmintic drugs (albendazole, mebendazole, levamisole and pyrantel) to all high-risk groups including school-age children and women of childbearing age twice a year can reduce local STH infection prevalence by over 70% within 2 years, effectively cutting down the chronic transmission chain in communities. This large-scale deworming campaign has been proven to significantly lower the incidence of STH-related anemia, childhood growth retardation and cognitive development impairment across endemic areas.

 

Personal protective measures form the last line of defense to block individual STH infection routes. Wearing closed-toe shoes during agricultural work or outdoor activities completely prevents hookworm larvae in contaminated soil from penetrating bare foot skin, which is the primary transmission pathway for hookworm infections. Strictly following personal hygiene habits including handwashing before eating, drinking only treated clean water, and thoroughly washing or cooking raw vegetables grown in contaminated farmland can block the oral ingestion route of STH eggs, further reducing individual infection risks.

 

Integrated multi-sectoral interventions combine all the above measures to achieve sustainable STH elimination. Local public health departments, education institutions and agricultural management teams can collaborate to build improved sanitation facilities in rural communities, incorporate STH prevention knowledge into primary school health curriculums, and promote standardized fecal treatment practices in livestock farming. WHO 2025 epidemiological data shows that regions implementing this integrated control strategy have reduced STH prevalence to below 1%, meeting the national elimination standard for soil-transmitted helminthiasis.
